Pre-inquest review into the death of Dan Kay, 45

The inquest into Dan’s death has been adjourned pending a serious incident review by Mersey Care NHS Foundation Trust.

Dan worked for the Liverpool Echo and was a well-known campaigner on behalf of the families of the Hillsborough disaster victims. He had worked closely with the family of Anne Williams co-writing her daughter Sara Williams’ book With Hope in her Heart.

Following Dan’s tragic death there were many tributes and he was awarded a posthumous doctorate from Liverpool Hope University.

The care and treatment Dan received from Mersey Care NHS Foundation Trust’s Liaison Psychiatry Team and Crisis Resolution Home Treatment Team will be explored at the inquest.

At the PIR Assistant Coroner Mr Joseph Hart has advised he will direct Mersey Care, Dan’s GP and British Transport Police to provide witness statements and documents.

Dan’s family is represented by Leanne Devine, partner at law firm Leigh Day, who has instructed Dr Cian Murphy of Doughty Street Chambers. Both are members of INQUEST Lawyers Group.

Amos Waldman, Dan’s cousin said:

“We are pleased that the coroner is including within his scope Dan’s mental health care in the community and the involvement of Mersey Care NHS Foundation in the days leading up to Dan’s death. We believe there are important issues to be explored.

“We are determined to raise awareness around mental health care and the treatment provided to people in need. Dan’s friends and family are raising funds in support of various charities including running the Liverpool half marathon this Sunday 17 March in aid of the men’s mental health charity Tough to Talk."
